Getting Started
Before scheduling, I ask that you complete a short questionnaire to determine whether my practice fits your needs.
The questionnaire takes about five minutes to fill out right in your browser, and nothing you type is stored anywhere. When you finish, tap Save as PDF at the bottom of the form and email the file to me at the address shown on the form. Once I receive it, I will reach out with next steps.
Questionnaire for a child or teen, under 18 (completed by a parent or guardian)
Questionnaire for a young adult, 18 or older (completed by the young adult)
A note on privacy: email is convenient but not a fully secure way to send health information. If you would prefer not to email your questionnaire, use the Contact page to send me your availability (days and times) for the next 10 days, and we will set up a phone call instead.
Telehealth across state lines
I am licensed in Connecticut and hold a PSYPACT authorization, which lets me provide telehealth to clients who are physically located in a PSYPACT participating state at the time of each session. What matters is where you are during the session, not where you live. If you will be traveling to a state that does not participate, let me know ahead of time and we can plan around it.
Fees and insurance
I am an out-of-network provider and do not bill insurance directly. Payment is due at the time of service.
Initial consultation for adults, 75 minutes: $460
Intake for children and teens, including a parent interview, 90 minutes: $550
Individual therapy, 50 minutes: $305
Parent or family session, 50 minutes: $305
I hold a limited number of reduced-fee spots. If cost is a barrier, please ask.
If your plan includes out-of-network mental health benefits, I can give you a superbill to submit for reimbursement. Before your first appointment, it is worth calling your insurer to ask whether you have out-of-network outpatient mental health benefits, how much of your deductible has been met, and what percentage they reimburse for these CPT codes: 90791 (initial evaluation), 90834 (individual psychotherapy), 90846 (family psychotherapy without the client present), and 90847 (family psychotherapy with the client present).
In some cases, an insurer will agree to a single case agreement, which allows me to bill them directly for your care. These agreements are entirely at the discretion of your insurer, usually when they do not have an in-network provider with the specialized training you need, and I cannot guarantee one. If you would like to ask your insurer about it, let me know and I can provide the information they request.
If you are paying me directly, including if you plan to seek out-of-network reimbursement, you have the right to a Good Faith Estimate of expected costs before starting care.
